Sealed quotations are invited from the intending reputed firms/Authorised distributors/ Medical
agencies for supply of Drugs & Medical consumables of reputed Brands and from registered pathological and diagnostic centres to provide services (Pathological, X-Ray, Ultra Sound, ECG) at Mela site as per terms and conditions. The detail list of medicines etc. with terms and conditions are available at the district NIC website: www. ganjam.nic.in (CDMO link). The quotationers regarding Medicines and medical consumables are required to submit the VAT clearance certificate, valid Drug licence, PAN card and E.M.D Rs. 5000/- in shape of DD/Bankers Cheque in favour of Zilla Swasthya Samiti (ZSS), Ganjam, Berhampur payable at Berhampur along Specification for stationary article, printing of IEC materials, tent house, etc. are available in web site www.ganjam.nic.in. The quotations should reach the office of the undersigned by 14.02.11 at 3.00 pm either by Registered Post or Speed Post which will be opened on 15.02.11 at 11.00 am in the office of the CDMO, Ganjam in the presence of the quotationers or any of their representatives. The rate quoted for the items should be inclusive of all taxes and F.O.R destination.
